Camper Forms are downloadable on this site and at the MySportFit site as well. Camper Forms must be completed and submitted before your child can be left with us for camp, so please take care of this early. Since some specific insurance, doctor and special conditions information is requested and it's better if parents complete this information prior to the first day of camp.
